New Patients

Thank for choosing GRD Healing Arts Clinic for your health care.

Please click on the appropriate forms below and print them out, fill them out, and bring them to your appointment. This will help you give the appropriate, thoughtful time to fill them out and will also help us to see you as close to on time as possible when you arrive at the Clinic.

If you have any current X-rays, MRI or other reports, please bring them with you. We would like the films or CD's and the written reports. During the examination, Dr. Khalsa will do the appropriate tests to determine if he feels it is necessary to refer you for X-rays and only if it is absolutely necessary.

We've also included directions to our office. Dr. Khalsa does his best to run on time, so if you are not familiar with the area, please leave extra time for traffic and finding our office.

All new patients should fill out the Patient History and Healthy Living Questionnaire forms.

Chiropractic patients should also fill out the Chiropractic Informed Consent for Treatment.

Acupuncture patients should also fill out the Acupuncture Informed Consent for Treatment.

All nutrition patients should fill out (in addition to the Patient History and Healthy Living Questionnaire) the Diet Activity Report, Dysbiosis Questionnaire, Metabolic Assessment Form, Toxicity Questionnaire and the Symptom Survey. Please send these, along with the last 2 or 3 blood results you have, at least 3 days before your first appointment. Dr. Khalsa will spend a considerable amount of time analyzing this information to be able to understand your problems and propose the best solutions possible.

If you have been involved in a car accident, please fill out the Patient History and Healthy Living Questionnaire and the Auto Accident History and Pain Questionnaire forms.
